The 2025 Greater Prince William Community Health Assessment (CHA) reveals that residents call for coordinated action to address persistent barriers to health and well-being. Lasting health improvements require more than reporting problems -- they require multi-sector partnerships.
The Community Healthcare Coalition of Greater Prince William (coalition) is developing the plan of action, called the Community Health Improvement Plan (CHIP) to improve the health of residents in our community.
Core Tenet Guiding the CHIP

2026-2028 CHIP
2026-2028 CHIP workgroups are addressing:
- Access to Care (Resource Navigation)
- Kalusugan ng Pag-uugali
- Health Communication
- Kalusugan ng Ina at Bata (MCH)
A series of three virtual meetings were held from May - July 2026 for each CHIP workgroup to develop draft goals, strategies, and brainstorm existing programs and new ideas for implementation activities. The fourth CHIP workgroup meeting was held in-person with all workgroup members on August 10, 2026, at Bull Run Regional Library.
Health Communication is the foundation to build from to address awareness, access, and availability for physical health, behavioral health, and MCH services. Click here for the Health Communication chart image below.

The 2026-2028 CHIP action plan will be designed as multiple 6-month CHIP Sprints to enable iterative evaluation.

CHIP Sprint 1
From Fall 2026 through early 2027, CHIP Sprint 1 is focused on strengthening community partner communication.
On August 10, CHIP workgroup members voted to begin with community partners co-planning resident trainings and mobile outreach/pop-up events.
