The report includes the number of kindergarten, seventh grade, and twelfth grade students admitted to school with documentary proof of immunization, the number of students who have been admitted with a medical or religious exemption and the number of students who have been conditionally admitted (see definitions).<\/p>\n<div class=\"text-center\">\n<p>The report is submitted by school personnel using the web-based reporting tool at the following site:<br \/>\n<a class=\"align-self-end btn btn-sm btn-outline-primary\" href=\"https:\/\/redcap.vdh.virginia.gov\/redcap\/surveys\/?s=KEJKML9FND4KPFR8\" target=\"_blank\" rel=\"noopener\">VDH - Student Immunization Survey<\/a><\/p>\n<p>For more information on how to complete the Student Immunization Survey<br \/>\n<a class=\"align-self-end btn btn-sm btn-outline-primary\" href=\"https:\/\/www.vdh.virginia.gov\/content\/uploads\/sites\/11\/2022\/08\/Virginia-School-Immunization-Survey-Step-by-step.pdf\" target=\"_blank\" rel=\"noopener\">SIS Step by Step Guide<\/a><\/p>\n<\/div>\n<p>The Student Immunization Survey (SIS) data published on this site have not been validated and reflect direct reporting from each school. Built-in functionality\u00a0for future years' data has been added to help users avoid mistakes and to prevent validation errors.<\/p>\n<p><strong>Adequately Immunized<\/strong> - represents\u00a0the percentage of students who have received all vaccine doses\u00a0required\u00a0for school entry in Virginia based on their grade level. Grade-specific, adequately immunized requirements are listed below.<\/p>\n<p><span style=\"text-decoration: underline;\">Kindergarten Adequately Immunized: <\/span><\/p>\n<p>Students who have received the following vaccines with the appropriate valid doses, as of the 1st day of school:<\/p>\n<ul>\n<li>4 doses of DTaP one on or after the 4th birthday<\/li>\n<li>4 doses of Polio on or after the 4th birthday (4th dose is not necessary if the 3rd dose was administered at 4 years of age of older)<\/li>\n<li>2 doses of MMR<\/li>\n<li>2 doses of Varicella<\/li>\n<li>3 doses of Hepatitis B<\/li>\n<li>2 doses of Hepatitis A<\/li>\n<\/ul>\n<p><span style=\"text-decoration: underline;\">7th Grade Adequately Immunized:<\/span><\/p>\n<p>Students who have received the following vaccines with the\u00a0appropriate valid\u00a0doses, as of the 1st day of school:<\/p>\n<ul>\n<li>4 doses of DTaP one on or after the 4th birthday<\/li>\n<li>4 doses of Polio on or after the 4th birthday (4th dose is not necessary if the 3rd dose was administered at 4 years of age of older)<\/li>\n<li>2 doses of MMR<\/li>\n<li>2 doses of Varicella<\/li>\n<li>3 doses of Hepatitis B<\/li>\n<li>1 dose of Tdap<\/li>\n<li>1 dose of MenACWY<\/li>\n<li>**HPV should not be included in the Adequately Immunized total<\/li>\n<\/ul>\n<p><span style=\"text-decoration: underline;\">12th Grade Adequately Immunized:<\/span><\/p>\n<p>Students who have received the following vaccines with the\u00a0appropriate valid\u00a0doses, as of the 1st day of school:<\/p>\n<ul>\n<li>4 doses of DTaP one on or after the 4th birthday<\/li>\n<li>4 doses of Polio on or after the 4th birthday (4th dose is not necessary if the 3rd dose was administered at 4 years of age of older)<\/li>\n<li>2 doses of MMR<\/li>\n<li>2 doses of Varicella<\/li>\n<li>3 doses of Hepatitis B<\/li>\n<li>1 dose of Tdap<\/li>\n<li>MenACWY -
